Yep, it's Moeka
I'm thinking that this is the first time that Moeka has ever been able to confront Miho though personally I have a theory that it's the fixation on the final death of Miho that is what's keeping Moeka locked in a coma.

As vis said, Miho is back. She was in mostly-dead-dreamland, lying in her bed from 150 years ago and talking to the spirit of a girl who has been in a coma for years. But Moeko giving Miho (and/or Miho's story) crap about dying yet again, combined with Yaku pleading for Miho not to die, seems to have brought Miho back.

Masamichi claims Miho had taken advantage of a bad situation. Miho's "perhaps" reply reads to me like she doesn't agree but isn't going to argue the point with him. I think her other comments show she was concerned about how unhappy and upset Erika's fans were. I think she tried to help, and it went bad.
It seems like magical girls are powered by the emotion/belief/will of their fans (much like how it powers the Miho's Story). Maybe Erika quitting like that was going to have a bad effect on the power that flowed to the real Moeko. We know that fan will never really died [580], and the fans stayed fans even after Erika quit. But maybe Miho didn't know that would happen, or maybe what she did after Erika quit helped that connection stay alive.
That's all speculation, of course. I don't think we have any real evidence yet. But that might be a way to tie the real Moeko back into that, and you'd think there has to be some kind of connection.
edit: Moeko's says in this comic that Miho has so much life she gave some to Moeko. Maybe character analogues are the conduit for the power of fan belief. The emotion of the fans feeds the Story, and the Story uses that to power the magical girls, idols, and "characters" that give the fans something to love. It's the circle of life.

I'm not sure it's clear even now. She could have meant Piro, Largo, Ping, or maybe even Yuki. The way she said it was meant for Masamichi to assume she meant Yuki, but Miho is often deliberately ambiguous like that. It lets her be honest while still tweaking people.
I think it was meant that way when Fred first came up with it. A stereotypical name for a magical girl. Now we know there's a real Moeko, so presumeably her parents actually named her that. I guess it's no weirder than some names that people give their kids.

Meimi, Moeko and Ririka could have been a gang of 3 magical girls (presumably the 'good' ones, with Stability affinity) (even though Meimi was/is a thief).
Miho could have been their opponent (even though Meimi refers to her as a friend) at some point, because of Miho's background with the Horde.
Maybe it wasn't Ririka's crush that she sacrificed her powers to rescue?? Maybe it was Moeko?? After a battle, so exhausted herself, only enough to not let her friend die.
Ririka said that Moeko was in a coma since before Erika gave her a voice (1500). Maybe as a result of a magical battle (with Miho?)
When Erika quit, fans were emotional and distraught.
Miho might have actually wanted to redirect the power of those emotions from Moeko to herself? And it ended 'badly' for Miho because it was all too much for her heart.
But she succeeded, in having Moeko not wake up.
I could totally see Miho influencing Sonoda Hitoshi to dump Erika, to leave Moeko's fans in hung state... and so to continue having Moeko imprisoned ...
The hostility of Moeko in the dream here also seems to confirm that - Miho won that battle and imprisoned Moeko's consciousness in the hung state somewhere.
Then Miho overdid (omg) it with the fans, and while recuperating in ASF, met Piro online. Piro's actions towards Miho have swung her around from that path, so much that she fell in love with him. But with the Horde on her tail now, she could not afford to drag him into this danger, so, she dumped him...

After re-reading the comic, I get this weird feeling that maybe Moeko is somehow Miho's daughter from the beginning: Born from an ill mother, a sickly child who fell into a coma, and is doomed to repeat a cycle of reincarnation as well. A wellspring of immense power, locked away, but serving as inspiration for others.

My theory is that Erika's career as Moeko's VA and the subsequent boom in fans was a unique case to Miho. Unique in the sense that emotional "power" was being generated, but the actual focus of it was not around to use it, being in a coma.
So, when Erika quit at the height of her career, leaving a giant mass of "power" with neither a "real" focus nor an "actor" focus to control it, Miho attempted to commandeer it for her own purposes.
Rather than Moeko's voice(or live action) actor, Miho tried to get everyone to think SHE was Moeko and take control of the "power". I'm guessing Miho tried to use the hope from Moeko's potential future "if she got better" to counter her personal fans' belief that corners her in a state of perpetual life/death. By making people want her to live after "dying", she could break out of her repeating cycle.
After it evidently failed, it nonetheless left Moeko's identity as a magical girl ever more present, more real than Moeko herself in the minds of the people. Which might have kept the comatose girl alive longer than anyone expected.

I'm of the opinion that Stability would be the entity interested in keeping a catastrophic grade magical girl https://megatokyo.com/strip/1108 securely in a coma. For that they'd need to keep Miho in a tightly wound bind death loop. I think their miscalculation was in assuming that Erika would be able to handle that strain and hold up her end as the Idol for that particular story.
When Erika walked away telling them it was all a lie Miho may have thought to change direction by letting the fans in on the entire story but that ended very badly....pirate queen and all that...
That's why Miho was so terrified about fans discovering any more about her than they already knew

Exactly, Miho and her story are the power source. Stability manipulates and exploits her to its own ends.
EDIT: as an example, suppose in the DCU that Lex Luthor discovered a technique to use krytonite shackles to enslave Superman and force him to do things that benefited the entire human race and the planet by cleaning up pollution, providing unlimited power etc.
