Maybe Piro is blending into CA territory as well. It's a trope (will ... not ... link) in VNs that the protagonist is fairly clueless; Piro seems to be picking that up.

Clueless Protagonist? There's a sense in which he's always been that, but so has Largo, of course. It's a viewpoint thing and always has been. And lest we forget, just a few short months (weeks?) ago Piro arrived in MegaTokyo as a clueless n00b. Character change takes time - lots of time...

A fan on another chat pointed out something interesting. Notice now Megumi's accent slips in panel three. She sounds a lot like Mugi. I'll bet she makes an effort to "speak proper", but it naturally comes out under stress.

Well now. Megumi is upset that Junpei hasn't been more obviously actively presently protecting her, she's got notions about him and her, and she certainly is from the same family/area as Komugiko, from how she speaks under stress and when angry at herself etc. Is she like niece Marilyn physically, or with ears and tail and so on. A mystery. But what's being viewed in the end panel; good or bad?
Not clueless, dealing with a sudden unexpected crazy change in the script and plot. But Piro verbally confirms again he's as confused by the incongruent behavior as it first seemed; he's carrying away the story finish as portrayed by Kimiko, but she's moving up kissing. Quite unlike the dearly departed, in a situation inapplicable to either a gone Miho or understanding empathetic coordinated Kimiko. Or what most anyone normal would be doing in the current environment. More than moving up though, she's not answering. Or stopping even as the situation and driving becomes more erratic, until the WHAM! forces its own change in vector. Perhaps a change in behavior, later. Maybe not.
So, it appears that whatever is going on isn't exactly Kimiko being Kimiko. Not being Miho either, at least not where she is dead and the story is over in a cathartic way for the audience. Which then Piro is still expecting the story to go to the end of get the body away and let everyone else move to believing and feeling that things are over finally once and for all as had been foretold and as is required. While Kimiko (or whoever this now is) has moved seemingly to another story, without bothering to tell Piro. She might not be able to. Compelled and such. Or?
Of course, it might be that what was being attempted wasn't the actual plan or goal, and this is exactly the way it was supposed to go. (If somebody is pulling the strings.) Or that it's just something randomly going wrong, that will be giving the fans actual tragic deaths next, as the car is crushed into a flaming pile of wrecked machinery, metal, glass and rubber. Hardly likely, but who knows what it might do next. Something else then. Yet whatever it turns out being, apparently the story isn't over regardless of who is supposed to be dead or not and what it's supposed to cause, and something very odd is going on with Kimiko right now.

Kimiko's line seems incredibly sarcastic, I can't seem to read it any other way. So I can't credit she was seriously doing that. Also so impractical.

Whatever it is hits the car from the side (sound effect is on Megumi's right; she throws up her arm to the right, not in front of her face; Kimiko is pushed up along Piro to Megumi's left), as opposed to Megumi running into something. But in the last panel she looks at it by looking straight up instead of to the right, so it's something pretty big, big enough to have come in from the right but now be basically in front of her. But it must have also been moving pretty fast, she doesn't seem to have seen it coming. Her look to me strongly suggests astonishment but not so much fear, as I would expect if the TPCD had finally caught her.
That all seems to fit very well with 4'33"'s theory. Not sure why it's spoilered when it's speculation but Quando in Roma

It would certainly fit the narrative of her trash-talking Junpei in panel 2 only to have him immediately appear. Also her look in the last panel is strongly suggestive of her "OMG he's so hot" look she gets when he's in front of her, see panel 7 of 1478 for example.

Hmm, I guess as a very distant second possibility... Yuki? After all, she needs to get her butt in gear if she is going to be part of the action this chapter. But then I am much less confident that panel 6 is the face Megumi would be making in that case... unless her "Huh? A schoolgirl?" face is basically the same as all her other "generally stunned" faces.

Now yes, Kimiko's in a weird place right now given that she's not dropping back out of "character" or whatever, and there's definitely some ambiguous innuendo at the very least happening, but Fred's also shown himself to be a master of stringing out sequences like this that seem to be dripping with hidden meaning and having it turn out to be a smokescreen for nothing at all.
Frankly Piro and Kimiko aren't even the focus right now, those panels are kind of a "oh yeah, and Piro and Kimiko are still here too" thing going on.
